To receive your approved credit card on the next business day via courier, fax your signed application form and required supporting documents to (65) 6352 8795 by 2pm (Singapore Time), Monday to Friday, (excluding Public Holidays). Supporting documents include:

	Salaried Employees		Commission-Based Employees		Self-Employed
NRIC (front and back)
3 months’ Computerised Payslip		or
12 months’ CPF Contribution History Statement		or		or
2 years’ Income Tax Notice of Assessment		Only latest ITNA is required

For Foreigners: In addition to the above documents, please provide

Your credit card will be couriered to the address stated on the application form. If the courier delivery is unsuccessful, the credit card will be mailed to the same address on the next business day.
